加油作業記載
1. 加油會議:(到美國港口前48小時內)
A bunkering operationt meeting in charged by C/E XXX was held at deck office for the personnel involved in bunker replenishment operation in LAS port, attendance persons ms、C/O、2/O、3/O、2/E、3/E、4/E、FT、BS、AB、AB、AB、MM、JMM.
The meeting for bunkering operationt was conducted by C/E,2/E,2/O,3/O and on duty G.P.C. at engine office on 1000-1100 to discuss the bunkering procedures.
The meeting agendas as follows:
(1) Pre- loading plan & bunkering procedures.
(2) Penalty of violation.
(3) Vsl‘s oil transfer procedure as per 33 CFR 155.750. Including each person’s responsibilities.
(4) Communication guide of standard, hand signals, and English phrases---both internal and external.
(5) Individual duty of person involved in bunkering.
(6) Emergency shut down procedures.
(7) Emergency stations for anti-pollution.
2. 加油船靠上:Bunker barge “XXX“ was made fast at port (starboard) side.
3. 加油前:Pre-transfer conference was conducted by C/E with bunker barge.
4. 開始加油:Commenced bunkering.
5. 加完油:Completed bunkering, total replenished: IFO 380/2500 MT; MDO 100.5 MT; L.O. (code) 20,000 LTRS, etc.

離港作業記載
在美國港口,根據33 CFR 164.25規定,須對下列項目做檢查及測試:
Within 12 hours before departure of the ship from port, following systems to be checked and tested:
1. Main steering gear and system
2. Auxiliary steering gear and system
3. The remote control systems of steering gear
4. The steering position indicator on the navigation bridge
5. The emergency power supply to one of the steering unit
6. The rudder angle indicators showing actual position of the rudder
7. Power failure alarms for the remote steering gear control system
8. Power unit failure alarms for the steering gear unit
9. Automatic isolating arrangements and other automatic equipment
Following listed procedure must be included along with the check and tests described above:
1. The full movement of the rudder as per the required capabilities of the steering gear system present onboard.
2. A visual inspection of all the linkages and connection in the steering gear.
3. The means of communication between the steering gear room and navigational bridge must always be operational.
1. 航前查船:
l According to the S.C.I.A. all crew mustered at partly gallery, C/O explained what to search and assigned everyone where to search by checking list. After the rummage. No smuggling. No contrabands found.
l All crew search through out the vessel for stowaway, but nothing to found.
2. 航前準備:Tested steering gear and M/E function found all good orders.
3. 備車離港:R.S.E. for Dep. from New York.
4. 拖船協助:Tug (name) attended (fore)/made fast (P‘bow、center lead).
5. 各纜打單:Single up.
6. 各纜解脫:All lines cast off.
7. 領港離船及拖船解散:Pilot left and tug dismissed.
8. 通過防波堤:Passed B/W.
9. R.F.A. at position L…λ…,Vsl. bound for “NFK” with full speed and set Co. on 090°(T).
各種作業記載
1. 污水排海:Discharged 11 M3 bilge water overboard through OWS equipment form L 41-15 N, λ 136-35 W to L 40-22 N, λ 133-58W, ship speed 19 knots.
2. 貨櫃繫固:Checked & adjusted lashing of containers loaded on deck, found all in secure condition.
3. 甲板檢查:All passages, watertight doors & man holes were property secured.
4. 安全檢查:Checked all D.G. goods, fire-fighting systems, tested ship‘s general alarm & safety shut down devices, found all in good order.
操演訓練記載
每月滅火及救生艇操演
1. 滅火操演:
Mustered all crew members to carry out the fire-fighting drill as following subjects:
(1) Reporting to stations and preparing for the duties described in the muster list.
(2) Starting of a fire pump, using at least the two required jet of water.
(3) Checking of fireman's outfit and other personal rescue equipment.
(4) Checking of relevant communication equipment.
(5) Checking the operation of watertight doors, fire doors, fire dampers and main inlets and outlets of ventilation systems in the drill area.
(6) Checking the necessary arrangements for subsequent abandoning of the ship.
The M/S debriefed the drill and was satisfied with the results.
2. 救生艇操演:
Mustered all crew members to carry out the life boat drill as following subjects:
(1) Summoning of crew to muster stations with the abandon alarm, followed by drill announcement on the public address system and ensuring that they are made aware of the order to abandon ship.
(2) Reporting to stations and preparing for the duties described in the muster list.
(3) Checking that crew are suitably dressed.
(4) Checking that lifejackets are correctly donned.
(5) lowering of at least one life boat after any necessary preparation for launching.
(6) Starting and operating the lifeboat engine for 3 minutes.
(7) Instruction in the use of radio life-saving appliances.
The M/S debriefed the drill and was satisfied with the results.
每兩個月進入密閉空間及救援操演
A enclosed space entry and rescue drill was carried out as following subjects:
(1) Recognition of the circumstances and activities likely to lead to the presence of a dangerous atmosphere.
(2) The hazards associated with entry into dangerous spaces, and the precautions to be taken.
(3) The use and maintenance of equipment and clothing required for entry into dangerous spaces.
(4) Instruction and drills in rescue from dangerous spaces.
The M/S debriefed the drill and was satisfied with the results.
每三個月棄船及緊急舵機操演
棄船操演:
Mustered all crew members to carry out abandon ship drill as following subjects:
(No.1 life boat lowered down into water and unhooked, crew practiced maneuvering boat)
(1) Summoning the crew to report to muster stations and prepare for assigned duties.
(2) Ensuring lifejackets are donned correctly.
(3) Demonstrating the operation of davits used for launching life boat
(4) Recover the life boat procedures.
The M/S debriefed the drill and was satisfied with the results.
緊急舵機操演:
A emergency steering gear drill was carried out as following subjects:
(1) All deckhands were mustered in steering gear room for drill.
(2) Tested communication between wheel house and steering gear room.
(3) Switch over the rudder power supply from primary power to emergency power.
(4) Maneuvering rudder in local site. (rudder angle do not over 15º for emergency power)
(5) Checked the rudder angle indicator align with the wheel house.
(6) Restore normal power supply.
The M/S debriefed the drill and was satisfied with the results.
SOPEP操演
A SOPEP drill was carried out according to the drills schedule. The emergency response for the situation of “collision/fire explosion/hull leakage/grounding/touching bottom and being towed” as per SOPEP.
The M/S debriefed the drill and was satisfied with the results.
急救訓練
Mustered all crew members to carry out the First-aid training as following subjects:
(1) First-aid treatment.
(2) Artificial respiration (including mouth to mouth & holger nielsen method).
(3) First-aid dressing(including 6 triangular bandages, 4 small standard dressing & medium standard dressing).
The M/S debriefed the drill and was satisfied with the results.
主機失靈及機側操俥操演
The M/E power failure, emergency stop & M/E local side control drills was carried out as following subjects:
Engine site:
(1) Inform bridge and change over control to engine control room.
(2) Put the blocking device on main engine governor to emergency position.
(3) Move the impact hand wheel to opposite position by turning it anticlockwise then disconnect the fuel pump from governor and connect it to regulating hand wheel on engine side control.
(4) Change over valve from remote to emergency control on engine side.
(5) Unlock the fuel regulating handle lock and move it to start position.
(6) From engine side telephones check the communications with bridge.
(7) On receiving order from bridge accept the telegraph and change over the valve to ahead or astern as required.
Bridge site:
(1) Check for other vessels and navigational hazards in vicinity.
(2) Broadcast SAFETY or URGENCY message as appropriate.
(3) Exhibit “Not under Command” shapes or lights and use sounds signals as necessary.
(4) Prepare for anchoring if water’s depth is suitable.
(5) Inform VTS or Port Authority and stakeholders.
The M/S debriefed the training and was satisfied with the results.
垃圾管理訓練
Mustered all crew members to carry out the garbage management training as following subjects:
(1) Categories of garbage.
(2) Garbage collection viewpoint & arrangement.
(3) Designated person & support staff duties.
(4) Procedure for dispose.
(5) Garbage record book & record-keeping.
The M/S debriefed the training and was satisfied with the results.
人員落水救援操演
A man overboard rescue drill was carried out as following subjects:
(1) Master start to perform a williamson turn.
(2) Throw overboard life-buoys and if possible self-igniting light and smoke signal.
(3) Keep your eyes on the person in water.
(4) Prepare jacob‘s ladder, safety nets and heaving lines.
(5) Stand-by derrick cranes and sling for lifting the person in water.
(6) Stand-by for using of the line throwing apparatus.
(7) Prepare first aid for immediate use.
The M/S debriefed the drill and was satisfied with the results.
防海盜及恐怖份子操演
Mustered all crew members to carry out the preventive actions against piracy and terrorism drills as following subjects:
(1) Increase the crew numbers for security patrolling.
(2) Block passageway and ladders leading to accommodation areas with rope.
(3) Make sure fire hose are rigged on deck and good water pressure is available.
(4) Sound the alarm warning when there is a threat of boarding by pirates or terrorists.
(5) Be especially vigilant in areas where concentrations of small fishing boats are present.
(6) Use search lights to illuminate and dazzle suspect craft. Most attacks are deterred if pirates or terrorists are aware they have been detected,
The M/S debriefed the drill and was satisfied with the results.
資淺船副的危險貨訓練
A training session for safe transportation of D.G. (IMDG Code including EMS & MFAG) to J/O Mr. XXX, was carried out and instructed by M/S & C/O.
The M/S debriefed the training and was satisfied with the results.
新進人員上船兩週內實施救生/滅火設備操作訓練及安全意識講習
(超過1/4船員更換,需於24小時內實施)
New crew members received life-saving/fire-fighting equipment operation training and safety awareness lectures, The M/S debriefed the drill and was satisfied with the results.
進入美國水域12小時前記載
1. 航儀測試:
l Tested all Navigational gears as per 33 CFR 164.25 &35, found all in good order.
l Test Primary & Secondary steering gears/internal communications/emergency generator/ batteries for emergency lights/general alarm & fire alarm as per 33 CFR 164.25, found all in good order.
2. 防毒品、防偷渡及違禁品搜查:
l Search around the ship as per SCIA for anti-drugs, contraband and stowaway by all crew members, but none of those were found.
l Nothing was found after all crew members searched all passages, cabins and common spaces for stowaway and contraband.
l According to the S.C.I.A. All crew members searched vessel’s space for smuggling drugs, stowaways & contrabands found nothing.
3. 甲板清洗:
l Swept and cleaned upper deck、passages、ladders、catwalk and hatches, and also rinsed it with fresh water jet. All working areas were free from grease/oil stain and safe for work.
l All working quarters had been swept/cleaned by ship‘s hands and suitable for cargo operation.
4. 壓艙水交換:At open sea, exchanged the ballast water of No.3U(P & S) and 5U(P & S) at position L 42-00N, λ 175-14 W.
5. 依據CFR 49裝載載危險櫃應記載事項:Inspected hazardous containers in stowage and found all in normal condition.
6. 到美國港口前,放大洋最後一個港口開航後應記錄:
Deck officers‘ conference was held by master for the Bridge Resource Management(BRM), the standing order and the voyage plan.
美國33 CFR 164.25規定記載事項
船舶在進入或航行於美國水域前12小時前,除非完成了以下設備的測試才能進入或航行於美國水域:CFR 164.25 Tests before entering or getting underway. No person may cause a vessel to enter into or get underway on the navigable waters of the United States unless no more than 12 hours before entering or getting underway, the following equipment has been tested:
檢查兩部舵機連接裝置是否良好,另外還需要做下列的操作測試:Primary and secondary steering gear. The test procedure includes a visual inspection of the steering gear and its connecting apparatus, and, where applicable, the operation of the following:

Each steering position located on the navigating bridge.
3. 主操舵裝置由備用電源供電(若有配備)。

4. 每個舵角指示器相對於舵的實際位置。(不得有誤差)
Each rudder angle indicator in relation to the actual position of the rudder.
5. 各遙控舵機控制系統斷電警報。(均需功能正常)
Each remote steering gear control system power failure alarm.
6. 各遙控舵機動力裝置故障警報。(均需功能正常)
Each remote steering gear power unit failure alarm.
7. 舵全速運轉能符合操舵裝置所要求的性能。

Visual inspection for life boats, life rafts and boat‘s davits, started life boat’s engine for 3 minutes and tested ahead/astern functions, found all good order.
2. 衛生檢查(MLC/A3.2.7)
The M/S & C/O conducted a sanitary inspection, including .crew‘s cabins, living quarter, galley, pantry and stores, and found all in clean condition.
